Program Submission Form

Thank you for your interest in working with Sachem Public Library. All potential programmers/performers must complete and submit the form below. 

Sachem Public Library receives a large amount of potential programmers weekly. We can only respond to those that fit the library’s interests and needs. 

Programs chosen will be contacted and scheduling will be done by the discretion of the library. Payment is made out by check only and sent out the month after your program. Payment dates are listed on contracts. If you’re not chosen, your information remains on file for future consideration.
