New 6th Grade Card
5th Grade Students use this form to request a new Teen library card for Sachem Public Library.
Please complete and submit this form by May 15, 2026.

**NOTE**
Teen cards are delivered to students’ 5th grade teachers at each elementary school for distribution.
Teen cards expire June 30 upon graduation from high school.
